OFCCP Revises Voluntary Self-Identification Disability Form

Contractors Must Update Forms by July 25, 2023

The U.S. Department of Labor’s (DOL) Office of Federal Contract Compliance Programs (OFCCP) has revised the Voluntary Self-Identification of Disability Form (CC-305) to update the preferred language for disabilities and to include additional examples of disabilities.

Federal contractors and subcontractors have until July 25, 2023 to implement the new form into their applicant and employee systems and processes.

The voluntary self-identification form is an OMB‐approved form. The only portion that contractors may modify or delete is the "For Employer Use Only" section, which was added to give contractors flexibility to enhance their recordkeeping for data analysis required by OFCCP's regulations.
