OFCCP Provides 503 Self-ID Form in Spanish, Word Formats

In response to requests from contractors, OFCCP posted to its website both the English and Spanish versions of the Voluntary Self-Identification of Disability form, required by the new regulations implementing Section 503 of the Rehabilitation Act (Section 503).  The form is available in .pdf and Word formats.  The English language version of the form was previously made available, although not in an editable format. Direct federal contractors are required to use this form to invite job applicants and employees to voluntarily self-identify as an individual with a disability as required by 41 CFR 60-741.42 of the new regulations. The regulations went into effect on March 24, 2014. The various versions of the form are available on the OFCCP’s website at http://www.dol.gov/ofccp/regs/compliance/section503.htm To learn more about complying with the new regulations, contractors are encouraged to read AGC’s Affirmative Action Manual for Construction, published after the rules were final.
